LensCrafters Vision Club	Your member ID card brings you special rates at LensCrafters. Your discount is available at all LensCrafters nationwide and may be used by all eligible family members. Receive a 20% discount on all eyewear purchases.
Crisp Learning	Members can take advantage of books and video/book programs on topics such as self-development, customer service, management training and other business/personal issues from Crisp Learning.
Customized Websites	Make your presence felt on the Internet with a customized website from eGroupManager. Their experienced staff of programmers and graphic designers will produce a site that you can be proud of.
Gateway Medicard	Keep your personal medical profile handy at all times. Gateway Medicard copies your personal medical profile onto a durable plastic card. Members receive one free medical card per account each year. Additional cards for family members are only $5 each.
Internet Access Services	In addition to PowerNet's great long distance phone service, association members can now take advantage of PNG's Unlimited Dial-Up Service for only $14.95 per month.
Vitamins & Nutritional Supplements	Members can obtain a wide range of vitamins, nutritional supplements, herbal remedies and generic over-the-counter medicines at discount prices.
Accidental Medical Expense	Underwritten by AIG. Benefit offers $2,500 primary and dependents, $100 deductible per member
Accidental Death & Dismemberment	Underwritten by AIG. Death: SelectSolution: $25,000/primary, $1,000/dependent SecureSolution: $10,000/primary, $1,000/dependent Specific Loss Both hands or both feet or sight in both eyes: Full benefit One hand and one foor: Full Benefit One hand or one foot and sight in one eye: Full benefit One hand or one foot or sight in one eye: One-half benefit
Accidental Disability	Underwritten by AIG. $250/week, primary only, Maximum of 52 weeks, 14 day elimination period.
Critical Illness	Underwritten by AIG. $2,500 benefit for primary and spouse only (no child coverage) Covers life threatening cancer; heart attacks; kidney (renal) failure; stroke; coma; coronary artery bypass graft; loss of sight, speech, or hearing; major organ transplant; paralysis; and severe burns. If life threatening cancer occurs in the first 90 days after effective date, benefit is reduced to $250. Coronary artery bypass graft benefit is $625. Paralysis and burn benefits vary.
Identity Theft Insurance	Underwritten by St. Paul Travelers. Up to $2,500 in financial relief if any member's identity is stolen. Includes reimbursement for costs associated with rectifying credit, lost wages incurred, and legal defense fees and expenses.
ID Network Child Safety Service	Pre-registry of children for peace of mind that photographs and descriptions are available for authorities when seconds count.
Discount Programs	Hearing - nationwide network of audiologists with up to 20% discount on services and up to 60% on hearing aids. Travel - 50% discount on over 5,000 hotels and motels in the United States. Also, a travel club to provide substantial discounts on vacation packages.
